Payment is due on the day of service. Payment methods accepted include: Cash, Zelle, Venmo, Major Credit cards, and Health Savings Account card.

Please verify with your insurance company prior to your appointment, if you will need a referral or prior authorization, to submit a receipt for reimbursement, for Out-of-Network services.
Services rendered are considered Out-of-Network for insurance claims. I will provide you with a detailed receipt, by request, to submit to your insurance company for possible reimbursement. Reimbursement rates can vary widely based upon your individual insurance plan, benefits and deductible. It is recommended that you call your insurance company, become familiar with your plan and/or check below with Reimbursify.
24-hour cancellation policy: Understandably, unforeseen circumstances sometimes arise that may prevent you from keeping your scheduled appointment. However, I kindly request you provide at least 24 hours' notice, if you need to cancel or reschedule your appointment, free of charge. Cancellations can be provided via email, phone call or text.
If you cancel or reschedule your appointment with less than 24 hours' notice, I reserve the right to charge you a cancellation fee equal to the full cost of your scheduled appointment.
